Is there a cost?
No. Members get these extra services at no cost as part of their Medi-Cal benefits.
How do i know if i qualify?
To get the ECM benefit, you must have complex needs that make managing your health difficult without additional support. Please contact us for further assessment.
What if i do not qualify for ecm but still need help?
We will do our best to refer you to the resources most appropriate for your situation.
How will this affect my current benefits?
ECM will not take away any of your current Medi-Cal benefits.
How long would i receive ECM?
You will be enrolled until your care goals are met or until you choose to disenroll. Participation is voluntary and you may choose to discontinue services at any time.
will this change my health plan?
No. There will be no changes to your health plan and you can see the same doctors.
